Sharing your information within the local health service in Lambeth
Your local NHS organisations in Lambeth have a duty to keep complete, accurate and up-to-date information about your health, so that you can receive the best possible care.
Sometimes the people caring for you need to share some of your information with others that are also supporting you. This could include GPs, hospital based specialists, nurses and health visitors.
To enable this information sharing to happen more quickly and to improve the care you receive, a new process has been put in place in Lambeth called the Local Care Record.
Through existing computer systems, this will join up care records held on your GP practice with the 3 main local hospital organisations, namely Guy’s & St Thomas’, King’s College Hospital and South London and Maudsley NHS Foundation Trusts.
Information is only shared when it is needed to make your care and treatment safer, easier and faster and only with those people directly involved in your care. This could include allowing a hospital doctor to see the medication that a GP has prescribed for you when you go into hospital or allowing a GP to see what care, tests or treatment you received while in hospital.
The doctor or healthcare professional treating you will inform you that they are accessing your shared Local Care Record. However, in an emergency situation where you may not be conscious or able to give consent, they will access your information to give you the best care.
Above all the Local Care Record will allow professionals supporting you to work with you to make safer and better decisions about your care.
To find out how to opt out (should you wish to) ask our receptionists to add read code "93C1" to your record; read codes are essentially flags with our computer systems. Shouldyou choose to opt out we must point out that this may effect the care you receive.

The purpose of this notice is to inform you of the type of information (including personal information) that Lambeth Clinical Commissioning Group (CCG) holds, how that information is used, who we may share that information with, and how we keep it secure and confidential.
What we do
The CCG is responsible for planning and buying (also known as commissioning) health services from healthcare providers such as hospitals, for our local population to ensure the highest quality of healthcare. We also have a performance and quality monitoring role of these services, which includes responding to any concerns from our patients on services offered.
How we collect and use information
We collect and use information to allow us to plan the provision of healthcare services. Most of the information that we usually collect and use is anonymous. In some circumstances we need your explicit consent to collect your information. We may also ask that the information from your healthcare providers, such as your GP and hospitals, is combined and joined up in order to better support you to stay well and for us to better plan the services we buy.As outlined in the NHS Constitution, we are committed to protecting your rights and responsibilities as a patient. These include your right to confidentiality and giving you access to your health information. You have the right to ask whether we have access to your information, and to choose that you do not want your information managed by us for certain purposes.

Confidential and identifiable data from your health record could be shared with the Health & Social Care Information Centre ( national provider of information, data and IT systems for health and social care )and approved third parties. We want to ensure that you are fully aware of all the facts so you can make an informed decision whether you want to limit the sharing of your data. This can be done by the use of what we call “Read Codes”, read codes are essentially flags with our computer systems. Below you will find a diagram about how your data could be shared and what read codes you can request be placed on your health record to prevent data extraction.

Medical Care when we are closed
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens.
NHS 111 Service: When our practice is closed you can still get support. You can get expert medical advice and useful ingformation by calling NHS 111 or through 111 online. The NHS service is available 24 hours a day and can provide information, issue prescriptions to a pharmacy of your choice, book a GP appointment, and, if necessary, refer people to emergency services.
Emergency: Dial 999 or go to your nearest A&E Department. Chest pains and / or shortness of breath do constitute an emergency (you must not dial 999 for anything other than an emergency).
Out of Hours Service: If you phone the surgery when we are closed you will be automatically transferred to the out of hours service SELDOC (South East London Doctors on call). Alternatively you can phone SELDOC directly on: 020 8693 9066. You will be given advice, asked to visit the out of hours centre, or be visited by the doctor on call.
NHS Walk In Service: The local NHS walk-in centre also provides healthcare advice, information and treatment with no appointment necessary. The nearest walk-in centre is: The Junction Health Centre, Arches 5-8, Clapham Junction Station, 5-8 Grant Rd, London, SW11 2NU. Telephone: 0333 200 1718.

Minor Injuries: We do not provide a minor injuries service. You should attend A&E or the local minor injuries unit at Guy's Hospital Urgent Care Centre.
